What do you mean add 1040? You do not seem to know that a Form 1040 IS your tax return form.  When you prepared a tax return, you prepared it on a Form 1040.  That is the form that will go to the IRS to report your income, deductions and credits, etc.  

 

Look at your 2019 Form 1040 to see it:
