Please clarify. What form did your husband receive? Form 1040 is the tax return itself.

Received a 1040 form? Look again at what he received. A Form 1040 is the form you use when you prepare and FILE a tax return. It is the form on which you enter all of your information and income for the IRS to process.

If you already filed, what you sent to the IRS was a Form 1040.

Did he receive something else---like some sort of 1099?
