I JUST CREATED A NEW 1099-DIV AND STARTED TO FILL IT IN AND NOTICED IT WAS FOR ONLY ME AND I NEED TO HAVE IT AS FORM FOR ME AND MY WIFE.  HOW DO I ADD HER NAME TO THE FORM I JUST CREATED?
