Do I have to submit form W-2c? Return has already been accepted. Changes are to HSA contribution, and do not change refund amount. Should I send in the W-2c?If so, where?

The W-2c is only correcting the original amount that was reported in box 12b(W). FAQs indicate that amending the return is not required because the corrections do not change the refund amount. Employer says to submit W-2c. Where do I find the correct address to send it?

You don't mail the W-2c alone to the IRS, the employer does that.  You would only send the W-2c attached to an amended return.  You probably don't need to amend as long as you didn't make excess contributions to the HSA, but there's a chance the IRS may send a request for clarification.
