Do I still include a forfeited state tax refund amount as income when filing my federal tax return for 2017?

I did not file my 2016 federal/state taxes in time to receive my state tax refund. So I forfeited that state tax refund.

No, you would not include it as income on your 2017 return. You would only include it as 2017 income if you had received it during 2017.
