I have been filing a CSA 1099 form instead of a regular 1099 R for some years now.  How do I correct this mistake?  Thank you.

That is not necessarily a mistake.   Form CSA 1099-R is the statement of Annuity Payments.

    • It's used to report civil service retirement payments and it is a variation of the basic 1099-R.
    • The CSA-Form 1099-R box numbers are the same as the numbering on a Form 1099-R.
    • Therefore, everything was calculated in the same way.

Thank you very much for your reply.  It's a big relief to know that there is no difference in calculations between the two forms.
