To complete Form 1040X, you will need:
Form 1040X ;
A copy of the return you are amending (for example, 2014 Form 1040), including supporting forms, schedules, and any worksheets you completed;
Notices from the IRS on any adjustments to that return;
Column A is what was originally reported
Column B is what is being added or deducted via the amend
Column C is the new total
EX: I reported an income of 21580 that goes in A, that total included a payment of 680 that I did not receive until January 23 so -680 goes in B, and in C I would put 20900
OR
I reported an income of 23450 that goes in A, another W2 came in for 3275 that goes in B, then in C I would put 26725.
Please remember that an amended return can not be efiled, you must print, sign and mail it to the nearest Dept of Revenue Office for your area.
Refunds from the amended return can not be direct deposited, a paper check will be mailed to the last address the IRS has on file for you.
