How can I print a supplemental list of contributions' to attach to my return?

is there a form to list contributions to attach to the Schedule A

such a list is not required. if the IRS ever wants it it will notify you.

    The IRS does not want attachments that are not required by the tax rules. The IRS instructions for Form 1040 say "Don't attach correspondence or other items unless required to do so." If the IRS has a question about your tax return or wants more information, they will send you a letter asking for it.
