How did my bank account info go missing from my tax forms? I filed with Turbo Tax and soon got letters stating payments weren't possible due to missing bank acct info.

I filed both state and federal taxes with Turbo Tax and chose to pay the taxes I owed via my bank account. I entered both routing and account numbers in the form. Days after filing, I got letters stating they couldn't process the payments. When I went back to the forms generated by turbo tax, the fields where the bank account info should be were blank. Now I have to pay a processing fee on top of my taxes.
