How do I amend 2019 form 8889 to change my HSA contribution from employer paid to employee paid.

my 2019 form 8889 (HSA contribution) shows employer paid when it was actually employee paid.  How do I amend this return.

It depends if the amount was reflected in W2 in Box 12 Code W.  If this is the case, then do not amend because Box 12 reflects employer contributions as well as employee payroll contributions.

I think I filled out the 8889 form from 2021 wrong. I didn't put any value in line 2 and I wrongly put a value in line 9 that said contributions were made from the employer.

If the contributions you’re describing were actually made directly to the account (and not from your checks at work), that would be a deduction you left on the table. And depending on the rest of your scenario last year, you could possibly get a refund now by “amending” that return (as discussed in this TurboTax article).

 

However, line 9 of that form includes not only what we would logically think of as “employer contributions,” but also your contributions deducted “pre-tax” from your pay (which is confusing, of course). So if that line 9 amount was also on your W-2 (with a “W” in box 12— see DaveF1006’s post above in this thread), then it was in the right place, and there’s nothing to do at this point.
