How do I amend my state tax return to include SSA-1099 benefits I forgot to include when I filed the original return?

You may need to amend your federal return. If you have taxable social security. If you amend your federal return and your refund or amount due is not affected, don't file it.

Whether or not your Social Security income is taxable depends on your total income, including your Social Security plus any other income. Generally speaking, if your only income is Social Security, you probably don't make enough money to be required to file a federal tax return.
