I am not getting 1099 misc or a W2 but I did get paid 633.00 extra working for Texas workforce rehab services as a job coach. Return filed as extra income, do I amend

I only worked for a couple of weeks as a job coach helping Special Ed kids on the job training.  I earned $633.00 extra income.  Texas Workforce Rehab Services did not and will not give me a w2 or a 1099 misc.  I filed my return and added the 633 as extra income.  Do I need to amend this return?  I did it online and didn't print out my return until now.  I noticed form 8919 printed out as an attachment with my return.  On line 1 of this form there is a  code H stating I received w2 and a form 1099 misc but that wages were not included in the 1099 misc.  I didn't and will not recieve a w2 or 1099 misc for this pay.  Do I need to amend? How?

I'd suggest that you not amend.  If the IRS needs any additional information, it'll reach out to you.  And with income as low as this with no corresponding W-2 or 1099-MISC on file, that's not likely.
