I have a loan on my 401k, but i did not take it out last year, i am just making payments back to the loan. do i need to report this?

A loan from a 401(k) tax deferred retirement account is not reported on a federal tax return.  If you default on the loan it then becomes a distribution and you would receive a Form 1099-R which would have to be reported on the tax return.
