Solved

I'm sending out my amended tax forms, and I just wanted to verify what needs to be sent.

On the what you need to mail section it mentions specific forms. but further down mentions other forms. Do I just send the amended tax forms stated in the first line or everything that was printed out when I amended them?  I assume I should include a copy of the 1099 that I had amended to my returns to both the State and Federal returns  that were affected.

Lastly, I moved last year and I don't have any changes on my new state return. It shows to return the amended form (that has no changes), but the next like states:

"attach the form (s) W-2 to the back of your return"  do I need to include my w-2 if there were no changes to it?

You will only include the information statements (e.g. Form W-2, Form 1099-MISC) that are related to the amendment. There isn't a need to send statements that were submitted with the original return.

Just to clarify those would be copies of the 1099 I had that needed to be amended into the original return, and the stated amended forms that need to be included ( 540, schedule X,etc)?
