I’m trying to receive my refund from last year but I got a form stating that I have to verify my identity? Is that why I didn’t receive my refund?

That is why. You need to comply with the request to verify your identity in order to receive your refund. Read the letter carefully. It should have a phone number on it, too, so if there is something it as asking for that you do not understand, call and find out what you need to do.
