I made an amendment with a late 1095a form. What do I need to send in with the amendment?

Do I need to send in a copy if my w2 and 1099 form along with the 1095a form and amended taxes or just the 1095a form and taxes? Also, since it only affected my federal do I need to send in th state return as well that was printed with the amendment?
