I need hep getting past the UTE 500 form can anyone help please

Can you clarify what state you are filing in and what the form title is? The only form I see that is close to that is Form 500-UET from Georgia which is for the underpayment of estimated tax.

If this form is being generated, you may be liable for a penalty for not withholding or paying enough in taxes during the year as you earned your income.
