I need to fill out a eic eligibility form from the irs ...and i dont know what form they mean i recieved a letter cp27 ?

I received a letter from the irs saying im possibly eligible to claim my adult son for eic ...it says i need to fill out a eic eligibility worksheet.i dont know what form they mean .if im eligible after i fill it out i have to send it back with the receipt from the letter plz help
