I received notice from the irs that I need to file form 8822 and resubmit a new form 4506

    8822 is for change of address. and 4506 is a request for a copy of a tax return.  if you did not request a copy of your tax return using Form 4506, call the number on the notice ASAP.  it might be that someone is trying to get hold of your personal data for purposes of identity theft.
