Should i file an amendment for my disability w2 for maternity leave? No federal taxes was taken out.

Are you saying you have a W-2 that was not included in your original filing? Then yes, you should file an amended return to report that income.  Disability income may or may not be taxable.  The fact that yours was reported on a W-2 means that it is taxable. The fact that no federal taxes was taken out, does not change that.  

 

On the Ohio return, You may deduct  Benefits from an employee's disability plan paid as the result of a permanent (or presumed to be permanent) physical or mental disability. You will get a screen to enter the amount of the deduction (box 1 of the W-2) if you qualify. You probably don't qualify.
