Where do I find information for a 8999t?

I did not receive any form like this and it will not let me submit my return until I fill out one more item which comes from this form.

Form 8889 is used to report contributions and distributions from an HSA account.

If you made contributions to an HSA or received a 1099-SA form for distributions, then you will need to revisit the HSA section to answer all the questions that flow to Form 8889. Be certain to indicate if you were enrolled in an HDHP insurance plan.

Where do I find the form 8999T

You would not have received a Form 8999-T.  Form 8999 -T would be created by TurboTax if you stated you has a HSA plan in the interview process.  The T stands for the Taxpayer.  This could of been indicated if you enter a code W in box 12 of the Taxpayers W-2 or during the HSA interview process.  Check the taxpayers W-2 entry for a code W in box 12 or follow these steps to check the interview questions:

 

  1. Click Federal
  2. Click Deductions and Credits
  3. Click Medical
  4. Click HSA and then HSA Contributions.
