Why does IRS say refund will be mailed

Direct deposit or mailed

for some reason, bad a/c number and/or routing number, something inconsistent with what's on the refund and what's on the account (like using a friend's account that does not have your name on it). The IRS may have tried direct deposit but it didn't go through so the refund went back to the IRS to issue you a paper check.
