Question

Why was I given a 1040-V when amending my tax return via e-file? Usually the money is deducted from my bank account

All I did was change the mailing address on my return. For the past few years, tax due has always been deducted from my bank account based on information provided via TurboTax. Why does changing an address necessitate sending in a check?

A change of address is not a reason to amend your tax return.  You do not send in a Form 1040X for a change of address.   All you need to do is send a change of address form if that was the only thing you wanted to change.
