The Form 568 generated by Turbo tax does show my SMLLC's name, address and other information even though I have entered them in the turbo tax worksheet. The top part of page 1 (side 1) was blank. Can anyone tell me how to fix this issue?
You have to enter it on the Schedule C (from the federal return) - not the Sch C worksheet. Enter your business name on Line C.
