Question

If I live in a state that collects income tax but work in one that doesn't, how do I enter box 15? Box 15 has state but no EIN, and 16/17 have numbers.

I live in Idaho and work in Washington. Box 15 lists ID for the state but no EIN (I'm guessing since the business isn't registered in ID). Boxes 16 and 17 have figures listed for withholdings.

The withholdings must be for your home state since there is no income tax in WA. You can enter the same federal EIN in Box 16. Box 15 should have Idaho.
