If the Single Member Limited Liability Companies (SMLLC) has not filed for any exception to be treated differently than the entity is reported with the personal return Form 1040.  The usual would be that the activity is reported on a Schedule C but if it is dealing with rental property the schedule E and if it is farming or related industries on a schedule F.

If the SMLLC wishes to file differently as a Corporation or Partnership , IRS Form 8832 Entity Classification Election.  IF you wish to file as a S Corporation you will need to file Form 2553 Election by a Small Business Corporation
