Puerto Rico form 480.6c for dividends and taxes withheld in a Roth Ira

I received this form for a Roth account. There are no taxes on the income in a Roth account? I am a non resident so 

1) do I have to report dividends as taxable ?

2) if yes to above then where do I report taxes withheld

    No.  Activity inside of a Roth IRA (or any IRA for that matter) is not taxable on a federal tax return.  You do not need to report this.

     

    However, you may have to file a return to get back your tax withholding.  You should contact the company managing your ROTH and ask them to stop withholding taxes.
