Schedule C: I don't have a business but I did get a 1099-MISC. What do I put for address and do I select yes or no for the 'Materially particiapte' question?

The income reported on form 1099-MISC is self-employment income and the work to do for that 1099-MISC is your "business". If you do not have a separate address for your "business", you use your personal address as the address on your Schedule C. You should say that you "materially participate in the "business".

thank you!  What about line I: Did you make any payments that require you to file a 1099?  I received a 1099 but not sure what to put here.

I don' t have a business   so don't have 1099-MISC. I work for a company name  Houston Home Care LLC   I'M a Employee
