The box numbers on our 1099-G form don't match your input form. examples fed tax w/h is box 3 not box 4; state income tax w/d is box 4 not box 11; there is no box 10b?

Sounds like yiur state sent a non-compliant 1099-G form that does not match the official Federal 1099-G.

Here:  https://www.irs.gov/pub/irs-pdf/f1099g_20.pdf

Enter according to the description then and not the box number.   Tax withheld must be in the official box in order for you to get credit for it.

Sadly not  all state forms conform to the IRS standard ... just read the description in the boxes on the screen and marry it up with the form from your state appropriately  ...  If you have state withholding then enter your state in box 10a, use the fed # for the state in box 10b and the state withholding in box 11.    If you have NO state withholding then leave all state boxes blank in the program.

Which state is it ???

The important thing to note is that you are entering unemployment income in the section of TurboTax for unemployment instead of the section reporting state tax refunds (which are also on a Form 1099-G).

As mentioned by macuser_22, match the descriptions instead of the box numbers.

In order to avoid a TurboTax error condition related to the state tax withholdings, you must enter information in boxes 10a, 10b, and 11.  Box 10a is the state name and box 11 is the state taxes withheld.  If your Form 1099-G does not show a state ID number for box 10b, then enter the Federal ID number shown on your form.
