I have been filling out medical expanses on the free version of the app for the last 3 years now you are charging $20.99???

This change in how Free works was made for the 2021 tax year (last year), and 2022 is simply following that. The only difference is that the price of Deluxe has gone up a bit.

If you were 25 under or last year, you would have also been able to file for free, but that promotion is good for any version of TurboTax. So, even if you were upgraded it would have still been free.
