In this case, both you and the father of the kids will have to input the 1095A Form. You can allocate the premiums of the policy between both returns.

On your Tax Return, once you have input all the information of the 1095-A you will mark the box that saysI shared this policy with another taxpayer who is not on my taxes”. This will allow you to allocate the policy premiums for the health care coverage. You will put the percentage that belongs to you, and the father of the kids will put the percentage that belongs to the kids, as long as between both it equals 100% allocation. You can also agree if you want to claim 100% of the premium, and the father can claim 0%.
