16 yo child depend. in 2018. Child has special needs and we paid for educational advocate assessment/testing. What form do I submit, or enter on 1040 for deduction ?

These would be deductible as medical expenses subject to the 7.5% of your AGI threshold.  You would need to itemize and your itemized expenses would need to be greater than your standard deduction in order to be able to claim them.

You can deduct medical expenses for diagnosis or treatment of a disease.  If your testing expense was just to assess an already diagnosed condition it may not qualify.  If you itemize and meet the 7.5% threshold to deduct medical expenses you can try adding that cost but be aware that it could be challenged if you are audited.
