Box 5 is blank on my Form 1098 for 2018 and in the summary section, on this form, it is listed the "Mortgage Insurance Paid" amount. Should I enter this amount in Box 5?

Yes, you can enter the amount in Box 5 if you did, in fact, pay the PMI.  But, do note that - as of today - mortgage insurance premiums are not deductible federally.  I say as of today because last year during February Congress reauthorized the deduction for only the 2017 tax year.  However, it has not happened for 2018 yet and it may not.
