Paper versions of this form should already be available with electronic versions available March 31 at the latest.

Your bank may have been confused and thought you were talking about 5498, which is a contribution record to your HSA.

If they can't furnish this form for you and you are sure that you took distributions in 2021, the safest route is to file an extension of your taxes so you can get all the forms required.

An extension to file is not an extension to pay. If you anticipate owing taxes, you should pay the full amount owed by April 18 to avoid any late fees and interest.
