Do I use all of number 12 amounts for HSA

When you enter your W-2, the amounts in Box 12, Code W automatically flow to the HSA contributions section.  Do not attempt to enter W-2 HSA contributions a second time when you are asked for "contributions you personally made" in the HSA section of TurboTax, or else they will be double-counted, and you will be subject to penalties if the doubling puts you over the yearly maximum. 

When you are entering the Box 12 amounts in the W-2 input, you can add multiple lines, so every item in Box 12 gets entered.  You do not need to pull out the HSA contributions in any special way.
