Do qualified medical expenses include health insurance out of pocket maximums paid from HSA?

No.  If the out-of-pocket maximums were paid from your Health Savings Account (which is a tax-advantaged account), then they cannot be deducted on your tax return.  That would be considered double-dipping by the IRS.
