For health care deduction, should I enter the amount on the 1099-SA (Box 1) or the amount on the W2 - 12d W?

The amount in box 1 of the 1099-SA is the amount that was distributed (i.e., paid) from your HSA for medical expenses. This is not a deduction, although it is not added to income if the entire distribution was used for qualified medical expenses.

The amount for code "W" in box 12 on your W-2 is the sum of the amount that your employer and you (through a payroll deduction scheme) contributed to your HSA. Once you have entered this amount on the W-2 screen, you do not enter it anywhere else.

You should not enter HSA contributions (whether through your employer or independent of your employer) under itemized deductions - the code W amount shouldn't be repeated anywhere else, and the amount of any independent contributions should be entered only during the HSA interview when you are asked for any HSA contributions that you made outside of your employer.

I am not sure what "health care deduction" you are referring to, but if you have an HSA, then go to HSA, MSA Contributions under Medical, and complete the interview so that the form 8889 is properly complete and you get credit for all appropriate deductions related to your HSA.
