Health Insurance Payments

I have a health insurance policy in my personal name. I have been paying the premiums from the company. On what tax return should I include the cost of the premiums - personal return or business return? If it is business return, do I just enter it as an expense under health insurance? Do I need to show this on the W2?

    Is the business an S-Corp and are you a waged employee of said company ?
