Healthcare insurance premiums reimbursement

Last year I left my job and I got  this bill from my former employer regarding the healthcare insurance premiums that they paid for me. It was a lot more than what I usually paid. They said they charged me the full premium. Can I claim it as a medical expense in my tax return?

Yes, you paid it with after tax dollars so it can be included with your medical expenses.

Thanks. But can I apply it to my 401k withdrawal? Because I used most of it to pay the bill.

You can choose the exception that allow you to subtract the amount that was over 7.5% of your AGI. Please see the screen shot below. You can also include it as a medical expense.
