Hi! I received a 1042-S income code 28 form for gambling winnings. Both Federal and State tax were withheld (I am resident). Should I also mail copies of my 1042-s form?

Is it necessary to list in “Other income” that this is from a 1042-S?

    1. It depends. You would only need to mail the form 1042-S  if you are mailing in your tax return with w2s, etc.

    2. Yes. Gambling income should be listed on a 1099-G but that has a lot of numbers that I doubt you could enter. This leaves filing it as miscellaneous other income with the label, gambling 1042S.
