why does it say tax free employer contribution when the money amount is both employee contribution and mine together? did I fill out something wrong?

    No, you didn't fill out anything incorrectly.  The terminology used for HSA contributions made through an employer's "cafeteria plan" whether made by the employer or deducted from employee's pay is to call them both employer contributions.   A personal contribution would be one you made directly from after-tax funds.
