I have a e1 visa of 5years; do I need to fill a fbar form?

The FBAR form is required only for individual U.S Citizens and permanent residents (those with a "green card".)  You would not be required to report your foreign bank holdings if you filing a tax return as a resident due to the substantial presence test in most cases.

 

In addition to individuals FBAR reports are required of LLCs, trusts, estates, domestic partnerships, and trusts and estates with foreign financial assets that exceed $10,000 U.S. at any time during the calendar year.
