Question

I paid for child care for my two children in 2019. I didn't receive a form from them, only a statement that has the total paid. Does it need to be split between them?

Yes, you should split the amount between your 2 children. Child care expenses are limited to $3,000 for one child and to $6,000 for two or more children.
