Question

I pay monthly out of my paycheck for a Protection HSA plan. I'm confused on what 1095-A form I would need and there is no digital form on my benefits hub.

Only taxpayers who are enrolled in Obamacare (have insurance through the Marketplace) will receive a 1095-A.   If you have health insurance through an employer, you don't have Obamacare and can skip (Answer NO) the 1095-A questions.

Your HSA contributions should be reported on your W-2, so you don't need any additional entries to report those expenses.
