You need to complete form 3520 and complete parts 1 and 1V of the form. Turbo Tax does not support the filing of this form but you will need to fill it out and attach it to your tax return.
Once filled out, your return would need to be printed and mailed according to the mailing instructions Turbo Tax will provide. Download a copy of form 3520 here to complete.
