If I have 40,000 in Medical Bills in 2015 But I did not pay them yet do I claim it in 2016 when I pay them or 2015 when it accrued?

    You may deduct medical expenses in the year you pay them, not when they occur. 

    Note: If you pay them with credit card, for example in 2016... but started paying back the credit card in 2017 - you are considered to have paid your medical expenses in 2016.
