If I have huge medical expenses incurred in 2021 but won't be paying them until 2022 how do I take the medical tax break?

You cannot deduct medical expenses until you have paid them.   Since you say you did not pay in 2021 you cannot enter the expenses on a 2021 tax return.   If you pay in 2022, you can enter them on your 2022 return--next year.

You can only enter the medical expenses you actually paid for with out of pocket funds in the year of payment.  If you did not pay these expenses in 2021 then they cannot be reported on the 2021 tax return.  If the expenses are paid in 2022 then you can enter those expenses on your 2022 tax return that is filed next year in 2023.
