If I stopped delivery driving Grubhub but continued delivery driving Doordash, then according the option "I stopped Delivery Driving in 2020: y/n" what's the answer?

The page is asking to know how long I've been doing this type of work. It has the option to select "I stopped Food delivery driving in 2020." I continue driving with one company, but stopped with the other.
*Will I fill this question out for each income source? (I.e., both Grubhub AND Doordash)

When you report this, you will claim income and expenses for this same business.  The only thing you will will report separately is if you get two 1099-NEC and you will combine these in the same business, Delivery Driver. You will not select "I stopped Food Delivery in 2020".

I did it 2021 but forgot to include

Do you mean you stopped driving in 2021 and forgot to include that on your return?  If so, there is nothing you need to do.  When you file your 2022 taxes, the self-employed income may show up but it can be deleted at that time.
