If you did not inherit the HSA answer NO and move on.

If you did inherit the HSA then the program wants to know if the original owner was your spouse or not.

If the original owner was your spouse, answer NO.
A HSA inherited from your spouse becomes a HSA owned by the survivor.  It is still treated as a HSA and can be used to pay qualified medical expenses without paying taxes on the distributions.

If the original owner was NOT your spouse answer YES.
A HSA inherited from anyone other than your spouse is no longer a HSA and the fair market value of the account is taxable income to the survivor in the year of the death of the original account owner.
