Is the average price for a charitable donation of securities to be entered per share or for the entire lot donated?

You will enter totals for the lots. For example you donated 100 shares of Intuit on 5/1/2019 and the price that day was $25/share. Your donation is $100 minus your cost basis. Your cost basis is what you paid for those shares originally. If you bought 100 shares of stock at $20 per share, and paid a commission of $15.99 total, your cost basis would be $2,015.99, the total amount you paid.
